Recovery package

The recovery package is made up of 4 elements which together can support you to self care and manage your condition from diagnosis of cancer through to recovering from cancer treatment.

Holistic needs assessment

An assessment where you answer questions about all areas of your life, about your needs and what's important to you. You'll then discuss this with a member of your healthcare team and agree together on how to deal with your concerns.

This is sometimes called your care plan and is written down. You can have a copy of the plan and it can also be shared with other members of your healthcare team.

Treatment summary

This is created by your healthcare team at the end of all or part of your treatment. You can keep a copy. It describes the treatment you've had and what to expect now that this part of your treatment has ended.

Health and wellbeing events

Find out about healthy living and where to get support in your local area. They're a good opportunity to meet other people in a similar situation to you.

Cancer care review

This usually happens 3 to 6 months after your cancer diagnosis and is a discussion with your GP, practice nurse or other health professional. It's a chance to talk about your concerns and any information or support that you may need. You may find it helpful to write down a list of things that you want to talk about before the appointment.
