The Social Prescribing Service

The Social Prescribing Service supports people to address a wide range of issues including weight management. It's provided by Viaduct Care.

You can access this service via your GP practice or you can contact Viaduct Care directly.

Stockport Triage, Assessment & Referral Team (START)

START can offer advice and support to help you make changes to your weight and feel more confident about making these changes.

ABL Health

Adult weight management service

ABL Health's adult weight management service provides a programme including one-to-one support and group programmes (virtual and in-person). As part of the programme, you'll complete an assessment and be offered support which meets your needs.

The programme covers topics such as:

  • behaviour change
  • diet and exercise
  • eating on a budget
  • your relationship with eating
  • how to understand food labels

Psychological support is also available.

Family weight management service

ABL Health's family weight management service, called FAB (Food Activity Balance), provides a 12-week programme of fun physical activity and healthy lifestyle sessions for families. It's a great opportunity to learn new skills and to achieve long term healthy eating habits for your family.

The programme supports families with children aged over 4 who are above the 91st centile. You can use the NHS BMI calculator to check.
